Liquid Courage Consent Form

I consent to having a tattoo artist perform tattoo artwork on my body. I understand the process of tattooing is time-consuming, will leave permanent impressions on my body, and the impressions may need periodic care and upgrading to maintain the proper clarity and color. I also understand that there will be some pain and discomfort involved, and there is a possibility of complications, including but not limited to infection, dizziness, and fainting. Having fully understood the statements on this consent form, and have had full opportunity to discuss the procedure with my independent physician, I agree to have the tattoo application procedure performed. Additionally, I agree to hold my tattoo artist at Liquid Courage, L.L.C. free and harmless for any claims, demands, or suits for damages from any injury or complications that may result from the application of any tattoo artwork on my body.
